22K97 (MD) requisition forms were requisition forms used by the Republic Mobile Surgical Unit by 20 BBY during the Clone Wars. The forms could be used to requistion medical droids too be sent for use in the surgical units during the war. The Human surgeon Jos Vondar filled out multiple 22K97 (MD) forms in an attempt to gain two more medical droids for his staff during the Battle of the planet Drongar. Each attempt however was unsuccesful.^[1]

22K97 (MD) requisition forms were first mentioned in MedStar I: Battle Surgeons the first book in the MedStar Duology, written by Michael Reaves and Steve Perry, and published in 2004.^[1]
