What do Imperial Records say about whatever happened to Anakin Skywalker? Sure, the Emperor knows, along with a few select Imperials and a few other people across the Galaxy, but whatever happens to "Anakin"? Another dissident Jedi killed as apart of the "Jedi Rebellion"? He was recorded as dead, I think. Obi-Wan was paying attention to those who had been killed in his early days on Tatooine and he had no idea Anakin was alive—being there on Mustafar—until he saw Darth Vader on TV in The Last of the Jedi: The Desperate Mission and put two and two together.
