The Handmaiden Sisters were the six daughters of the Echani general Yusanis, who served Jedi Master and historian Atris during her time on Telos after the Jedi Civil War.
The eldest five were identical, as all Echani children with common parents usually were; the sixth and youngest of them, however, bore the face of her mother, Jedi Knight Arren Kae who fought before and during the Mandalorian Wars.
The sisters attended on Atris, and were all highly skilled in the martial arts of the Echani. Their primary weapon was a simple quarterstaff known as a Handmaiden's Staff, which could collapse into its handgrip. The sisters were also skilled with other close-quarter weaponry as well as in hand-to-hand fighting.
The sisters had all sworn an oath to never learn the ways of the Jedi. Their intended purpose at the Academy that Atris established was to be objective guardians of the Jedi, watching them for signs of dark side corruption and intervening as necessary, but the true reason is that Atris feared that she had fallen to the Dark Side, and she chose the sisters, who cannot feel the force to accompany her, so that they would not discover the taint of the Dark Side on her.
However, Brianna, the youngest sister, was always curious about the Jedi relics that Atris kept and longed to know what it felt like to use the Force. Because her mother was a Jedi, she was Force-sensitive even though her half-sisters were not.

Although dialogue in Star Wars: Knights of the Old Republic II: The Sith Lords indicates that Brianna looks different than her sisters, they all use the same textures in the game, although Brianna's model is different as it has beaded tails on either side of her head. However, when the Jedi Exile is in Atris' academy, she uses the normal Handmaiden model.
If the player is male, Brianna will join him as a party member and can then learn the ways of the Force. For this she will be seen as a traitor by her sisters, and she will fight and kill all five of them near the end of the game.
