Omega Red was a deadly Sith toxin created by Darth Maladi, based on Alpha Red. It was capable of killing any kind of species. She offered to reveal its secrets to Emperor Roan Fel in exchange for safe haven and Cade Skywalker's life. Roan Fel agreed to the deal but it turned out that the Sith were immune to the toxin by design as a trick by Maladi and Krayt to wipe out Coruscant. It was almost used by Roan Fel until Antares Draco killed him to prevent it from being used.
