Revan Edit

I saw this and decided to delete it since it is not confirmed in any source that he used the ability or even lived after the foundry encounter to teleport away to use it.

I have re-added the (Possible) use of Revan as a practitioner and have also added a more elaborate possibility of whence he used the technique. I bracketed it as Possibility due to the unconfirmed belief of him using it.
