This is not originally my screenshot, but I did fix the aspect ratio. Though it would have been really funny to see Dan Wallace explain the flat version.
Kuat on page 75, clearly based on my screenshot. Which had already been swiped by CSWE at that point, so no surprise there.
Anaxes on page 39 is derived from the Coruscant and the Core Worlds version.
Anoat on page 40 is from the CCG.
Bothawui on page 46 is a redraw of the NEGAS version, which is itself directly cribbed from the CCG.
Brentaal on page 48 is from CCW, and eschews the EAW version.
Byss on page 49 goes all the way back to the source.
Christophsis on page 50 was easily duplicated from TCW itself.
Csilla on page 53 recycles the NEGAS version, which is pretty consistent with the Fact Files version.
Duro on page 57 wisely ignores the NEGAS and goes back to CCW.
Eriadu on page 59 is a good interpretation of GATORW.
Felucia on page 60 is straight from TCW.
Gamorr on page 62 is recycled from NEGAS.
Iego on page 67 is, surprisingly, more consistent with GATORW than TCW.
Pre-Vong Ithor on 69 is pretty much like NEGAS with a little extra definition.
Same with Kalee on page 70. NEGAS, just sharpened.
Korriban on page 74 goes back to KOTOR (and Jedi Academy) and skips the TOR and Fact Files versions. Good.
Malastare on page 76 is somewhat like Gamorr and Ithor but adds more detail to the NEGAS version.
Dac on page 79 skips over the NEGAS version that had way too much land and goes back to GATORW.
Mygeeto on page 81 sticks to the only other appearance in Battlefront II.
Nagi on page 83 stays with the general concept from NEGAS but adds color.
Neimoidia on page 86 has similarities to the NEGAS version but actually subtracts color variety.
Nubia on page 87 is pretty much like the CCW version, just better defined.
Lehon on page 92 skips the inaccurate NEGAS rendition and goes back to the source.
Raxus Prime on page 94 is Raxus Prime from TFU. Whether this is a good thing I cannot say.
Rhen Var on page 95 is straight out of the Fact File. Probably not coincidentally, this is the only space shot we have of it.
Rishi on page 96, oddly, seems more like the generic Rebellion planet than the TCW version.
Ruusan on page 98 is exactly the same situation as Rhen Var.
Taanab on page 104 is an interpretation of the only view we've had before, from Jedi Academy. Aren't you glad someone extracted it from the game?
Thyferra on page 107 is a straight-up redraw of the Fact File version.
Triton on page 109 is straight from Rebellion.
Zonama Sekot on page 113 is the Fact Files version, expanded and spruced-up a bit.
