• I apologize; I forgot to load this information earlier. I did check Fightsaber and while it never says explicitly that "Anakin mastered Form V", it does mention his great level of skill (Pages 35-36 of the Insider edition are what I'm looking at), so I feel comfortable leaving it—the difference is largely semantic, if you ask me, given that Anakin was one of the best lightsaber duelists of his era.
